If the resource is a person: First enter the resource’s name and title in the fields below. Then select a name configuration in the drop-down.
If the resource is not a person: Enter the entity’s name here.

Do not enter the resource’s city, state or country. Instead, enter the resource’s postal code (zip code) in Zip, then click . RB9 will automatically populate the other address fields.

If the resource wants paychecks and statements sent to a different address, click to select Yes and enter the details below.
